A large part of the care provided by Interventional Endoscopy Services Medical Group (IESMG) is to help our patients arrange for care with our team of specialty physicians. Since our doctors are very specialized, often the first step is for the patient to obtain a referral from their physician. Our Administrative team is available to work with you to facilitate obtaining a referral should your insurance carrier requires one in order to be seen by us.  Whether or not a referral is necessary or provided, the next important step is to have your medical records pertaining to your particular diagnosis faxed to the Administrative Office.  The fax number is 415-375-4861.

Once your records are received, our IES physicians will carefully review them and determine what treatment is needed. Our Administrative team will then reach out to your insurance carrier to determine eligibility and if prior authorization for the procedure is needed. In other cases, if you are a self-pay or cash-pay patient an estimate of fees will be determined and payment will be required before your procedure. Once this is accomplished our office will work with you to make certain all details are addressed and will assist in setting a date and time for your procedure(s).

Important:  The records that we MUST receive in order for one of our physicians to review your case are:

  • History & Physical from Primary Care Physician
  • Progress note(s) from your referring physician (if not your primary care physician)
  • Prior endoscopy/colonoscopy/ERCP/capsule endoscopy (where applicable)
  • Radiology, pathology or other tests relevant to diagnosis
  • Lab work if done within the last 2 weeks
  • Insurance card and all forms completed
